• Annual statements of overall income (form ADP010) of the years subject to the refund application;
• Copy of National Identity Card;
• Certified copy of the contract of subscription to a supplementary pension;
• Certificate of payment of contributions: this certificate states, for contracts signed starting from January 1st 2009, that the insured person has opted for the deductibility of contributions or premiums;
• Income certificate for public sector employees or income certificate approved by taxation services for private sector employees or pension certificate;
• Bank account certification.
